Miscellaneous Services- Issue of Police Clearance Certificate or Surrender Certificate or Global Entry Programme verification or any other Miscellaneous certificates based on the Passport : $ 40 + $ 2 = $ 42
There shall be discount of 10% in passport fee for fresh applications (and not re-issue) in respect of minors up to the age of 8 years and Senior citizens (persons above the age of 60 years): Minor Fees: $ 81/- + $ 2/- = $ 83/-

In addition to the above fee, an amount of US $ 2/- is also payable towards the ICWF fee

NEW PASSPORT IN LIEU OF LOST PASSPORT

Requirements

1. All particulars in the application should tally with those in the lost passport.

2. Photocopy of the lost passport.

3. 5 recent photos with white background and frontal view. The length of face in photo should be one inch

regardless of size of whole photo/ no Photoshop/ no glasses/ no shadows/dark color shirt/very high

resolution.

4. Copy of the First Information Report (FIR) from the Police authorities and its authentic legal English

translation.

Original with a copy of one or more of the photo-affixed identity cards to establish identity and

nationality(The original will be returned after verification):-(a) Identity Card (b) Labour Card (c)Health Card

(d) Driving License (e) Election commission card.

5. The circumstances under which the passport was lost;

6. Details of travel on the lost passport; certifying that the applicant has not acquired any other nationality.

7. Details of travel concessions including Transfer of Residence benefits, Foreign Exchange availed of, imports

8. made by the applicant on the lost passport; certifying that the applicant has not acquired any other

nationality.

9. Places of stay and work in Baghdad since arrival

10. Personal Particulars Form (in Triplicate with photos pasted) in the case of passports issued by other

Passport Issuing Authorities.

11. Personal appearance of applicant is compulsory to establish physical identity.

NOTE: IN THE CASE OF APPLICANTS WHO ARE ON SHORT VISIT TO BAGHDAD AND NOT A RESIDENT IN THIS COUNTRY, HE/SHE WILL BE ISSUED AN EMERGENCY CERTIFICATE TO RETURN TO INDIA.
